Hormone Optimization: Why Your ‘Normal’ Labs Don’t Tell the Whole Story

A functional medicine approach to hormone optimization.

Here’s a conversation we have with patients quite often: They’re exhausted despite sleeping eight hours. Their brain feels wrapped in cotton. Simple decisions feel overwhelming. And when they finally get the courage to see their doctor about it, they’re told their bloodwork is “normal” and maybe they should try getting more sleep.

Sound familiar?

At Ivalo MD, we see this pattern play out a lot. “Normal” lab ranges tell us you’re not sick enough for a diagnosis, but they don’t tell us whether you’re actually thriving. There’s a significant difference between “not diseased” and “optimally healthy.”

Your hormones could be the missing piece of the puzzle.

While standard tests may technically fall within range, hormone imbalances (particularly involving thyroid, cortisol, and sex hormones) can derail your energy, focus, and mood long before they show up as “abnormal” on a basic lab panel.

This is precisely where functional medicine makes the difference. We don’t just look for disease; we optimize for vitality.

Why does standard bloodwork miss key hormonal imbalances?

Here’s the truth about conventional medicine: Most doctors run basic lab tests that don’t give you the whole hormone picture. So, let’s talk about what’s often missing in the typical test:

🔬 Incomplete thyroid testing

If your doctor checks your TSH (thyroid-stimulating hormone), that’s a good start, but it’s only one piece of the puzzle. It’s like asking your car mechanic about your engine, but they only listen to your car without ever popping the hood. It’s useful, but it doesn’t tell us everything that’s going on.

A full thyroid test should include free T3, free T4, reverse T3, thyroid antibodies, and ferritin (iron stores) to uncover some of the issues that go largely unnoticed until it’s too late, like Hashimoto’s or iron deficiency that mimics thyroid symptoms.

🔬 Cortisol levels aren’t properly measured

Cortisol is your body’s stress hormone. It’s great in small doses (hello, fight or flight), but when stress is constant, cortisol can wreak havoc (hello, burnout, weight gain, and brain fog).

Standard tests typically only measure morning cortisol through a single blood draw, but that’s like taking a snapshot of a movie and claiming you’ve seen the whole film. Cortisol doesn’t just show up at breakfast—it has a natural rhythm throughout your entire day. Functional medicine takes a more thorough approach, using saliva-based tests like the DUTCH that measure cortisol at 4 different points during the day. This gives us the full picture of how your stress hormones rise and fall, helping us address the real root of your symptoms instead of guessing based on incomplete data.

🔬 Sex hormones are often overlooked

Your estrogen, progesterone, and testosterone do way more than regulate your cycle or help you build muscle. They impact your metabolism, cognition, bone health, and overall well-being. Yet, some doctors either skip testing them or only do a basic check.

When we test for hormones, we don’t just check the levels, we dive into the nitty-gritty, looking at hormone ratios, metabolism, and detoxification pathways. That’s all to say: We’re not just seeing if you have hormones like standard tests show (we already know you do!), we’re figuring out if they’re doing their job.

Especially when it comes to hormone optimization for women, it’s important to make sure they’re working for you, not against you.

The intricate connection between cortisol, thyroid, and sex hormones.

Here’s the thing about hormones: They don’t like to work solo. They’re a team, and when one player’s off, the whole game gets thrown off. For example:

  • Cortisol + thyroid: Chronic stress can slow down your thyroid, leaving you tired, foggy, and struggling to lose weight.
  • Cortisol + sex hormones: Too much stress can deplete progesterone (this is called the “cortisol steal”), making your cycles irregular and triggering daily PMS-like symptoms.
  • Thyroid + sex hormones: Low thyroid function can mess with your estrogen and testosterone, draining your energy and affecting your mood.

So what does this all mean?

Achieving optimal hormone levels doesn’t mean picking one hormone and fixing it. It means looking at the whole team and making sure they’re working together, in sync.

The functional medicine blueprint for hormone optimization.

Instead of throwing medication (like birth control, thyroid medication, or antidepressants) at the problem, functional medicine looks at the full picture and works to optimize your hormones naturally.

For example, our approach includes:

Optimizing nutrition. More than eating your greens (although we highly recommend that), we focus on supporting blood sugar balance, gut health, and hormone production with whole foods.

Managing cortisol and stress.  Whether it’s through adaptogens, mindfulness, or better sleep, we aim to regulate your cortisol so your body can finally relax and rebalance.

Supporting thyroid function. We’ll tackle any nutrient deficiencies (think iodine, selenium, zinc, and ferritin) that could be standing in the way of your thyroid doing its job properly.

Restoring sex hormone balance. We use targeted nutrition, herbs, and bioidentical hormones (if needed) to get your estrogen, progesterone, and testosterone working in sync.

Detoxifying excess hormones.  Your liver and gut play a huge role in clearing out excess estrogen and other hormones. We’ll support those pathways to keep your hormone balance in check.

Your symptoms are real. Your intuition that something’s off? Also real.

The difference between feeling “OK” and feeling vibrant often comes down to hormone optimization that goes beyond basic lab ranges. At Ivalo MD, we use comprehensive testing to uncover the imbalances that conventional medicine typically misses.
